How to Participate

If you are interested in our Outpatient Services program, please do the following:

• Obtain a doctor's prescription for therapy.
• Call the Overlake Outpatient Rehabilitation Center to schedule an appointment at 425-688-5700.
• Bring a list of medications you are taking.
• Bring your pertinent medical history.

Clinic hours are from 7:30 a.m. to 6 p.m., Monday through Friday.

The clinic is located at 1120 112th Ave NE, Suite 150. It is on the first floor of the East building. Underground parking is available and validated.
