Plan to arrive 10 minutes (15 minutes for new patients) before the start time of your appointment. Bring your health insurance card and personal photo identification with you. Please also be ready to provide payment for any deposits or co-pays. Once our team has completed your check-in, you will be asked to take a seat in our waiting room. Your healthcare provider will be notified that you have arrived.
If you have not been a patient at any Overlake location previously, plan to arrive 15 minutes prior to the start of your appointment to complete check-in and patient registration forms. You may also access forms online to print at home and bring with you to the appointment.
If you are a returning patient, depending on the reason for your visit, there may be additional forms that we will ask you to complete.
A member of our care team will bring you from the waiting room into the clinic, and may take your height and weight measurements. You will then be taken into one of our patient rooms. A member of our team may ask you a few questions to confirm the reason for your visit in addition to taking your vitals (temperature, blood pressure, etc.), and hand off your visit to the doctor or nurse practitioner.
Your primary care provider will confirm the reason for your visit, and ask you additional questions in order to direct you to the right care. If you have specific questions you would like to ask, plan to write them down ahead of time and bring the list with you.
Following the visit with your primary care provider, you will receive a visit summary as well as instructions for any follow-up care needed (lab work, diagnostic imaging, etc.).
Electronic Health Record
We will ask you if you would like to register for free, secure access to your electronic health record — MyChart — which is used throughout our Overlake network. This coordination means that no matter which Overlake location you visit, your care team can access your medical history, including medications and allergies, and you will not need to repeat paperwork at every visit.
As an Overlake patient, you can access MyChart to:
- View your personal health record.
- Schedule medical appointments.
- View your medical history and lab results.
- Request prescription renewals.
- Get health maintenance notifications.
- Ask non-emergent medical questions to your doctor.
