Please use your account number when making payments or speaking with Customer Service.
2. Account Summary
This is a summary of Charges, Payments, Adjustments and Total Balance.
3. Payment Options
Easy ways to make your payments online via MyChart or by phone.
4. Proposed Prompt Pay Discount
Save money by taking advantage of the offered Prompt pay discount if paid in full within the first 30 days from statement date and by the due date.
5. QR Code
Scan the QR code to make payments or to start a Financial Assistance Application.
6. Notifications
Account Status, important messages, reminders and special offers.
7. Sign up for MyChart
Use the activation code to sign up for MyChart.
8. Payment Coupon
If paying by mail, detach the payment coupon and place in the return envelope provided.
9. New Account or Visit Information
A description of the care you received will be listed under this heading. Includes charges, payments and outstanding balance.
10. Visit Date and type of Service
Detailed information specific to the date of service.
11. Total Amount Due
Provides a combined total of Hospital and Professional Services. This is the amount you owe.
