Overlake Medical Center & Clinics is partnered with Elevate Patient Financial Solutions® to assist patients without health coverage. This is a free service and will include helping you:
- Understand if you may qualify for programs to assist with your medical bills.
- Apply for assistance if a qualifying program is identified.
- Communicate with the county/state agencies through which you apply.
- Gather necessary paperwork.
- Follow submitted applications until a determination is made or other resolution is achieved.

- Proof of income
- Other documentation as requested by the programs for which you apply
As part of this service, you may receive phone calls or mail from ElevatePFS®.
Overlake Medical Center & Clinics will only share initial contact information with ElevatePFS®, at which point ElevatePFS® will work with you to obtain needed information. We will not share your medical record information.
