To schedule an imaging exam, call our centralized scheduling department (425) 688-5700 and fax your order form to (425) 688-5710. Walk-ins are welcome for exams that do not require preparation procedures (prior authorization is required for some exams). Your referring provider will need to complete following form and bring it with to your appointment.

To obtain a CD of your medical images taken at Overlake, please call (425) 688-5564 and select option 2 from the menu.
